PERSONAL SUPPORT SERVICES
Hands of Hope Services, Inc will use a variety of person-centered planning techniques to articulate the lifestyle choices of the individuals that we support. We realize that the most effective person-centered plans are those that are flexible and change to reflect the personal growth, increased independence and lifestyle choices that the individual makes over time.
Hands of Hope Services’ Personal Support services offer a range of home and community-based services intended to assist the individual to participate fully in-home and community life such as:
- Bathing
- Budgeting and Money management
- Toileting
- Care of Adaptive devices
- Developing Relationships
- Eating and preparing meals
- Dressing and changing clothes
- Home and Community safety
- Community resources awareness
- Light housework including laundry
- Promoting Self-advocacy and Self-Determination
- Promoting Self-advocacy and Self-Determination
- Moving, turning, and positioning the body while in bed or in a wheelchair
- Mobility, including transferring from a bed, chair, or other places, and moving about indoors and outdoors
Hands of Hope Services, Inc. Personal Support services offer a range of home and community-based integration service to assist individuals who live in their own or family homes with building skills essential to maximize their personal independence. These services include:
- Budgeting; assisting with money management, banking and tax preparation
- Completing homework
- Counseling
- Job Coaching
- Helping an individual to access and complete the participant’s education
- Participating in recreational and social activities;
- Accessing community services
- Grocery shopping and meal preparation
- Personal care and assistance with ADL’s
- Housekeeping and home management.
- Involve in leisure activities, as well as all aspect of living independently
- Respite, behavioral, and other services and supports needed by the family of the participant.
- Task to ensure health and safety, including Nursing services, medication administration and assistance with medical appointments.
- Assist in establishing relationships in the community with other, organization, and/or associations- community integration and engagement skills development.
Personal Supports are available:
- Before and after school;
- Any time when school is not in session
- Before and after meaningful day services (example Employment Services, Supported Employment, Employment Discovery and Customization, Career Exploration, Community Development Services, and Day Habilitation); and
- On nights and weekends.
