Our Supported Living Services will provide services to individuals in their own house or apartment of their choice. Hands of Hope Services, Inc., will ensure that services include provision of coordination, training, support, and/or supervision as indicated in the individual’s Person-Centered Plan. We will assure that all staff acquire relevant DDA trainings in all levels and pattern as determined in the individual’s Person-Centered Plan. Our Supported Living Services enhances the individual’s opportunity for community participation, independency, making decision on where to live, and exercising choice and control over his/her own life.

Hands of Hope Services, Inc., will offer a variety of individualized services to support living independently in the community in the southern and central regions. Hands of Hope Services, Inc., will provide supported living services that are individualized to the individual’s needs and interests as documented in their Person-Centered Plan and must be delivered in a personalized manner.

Supported Living services will assist the individual to:

• Learns self-direction and problem-solving related to performing activities of daily living and instrumental activities of daily living required for the individual to live independently; and
• Engages in community-based activities of the individual’s choosing within the individual’s personal resources.

Hands of Hope Services, Inc., Supported living services enhances the individual’s opportunity for community participation, independence, making decisions on where to live, and exercising choice and control over his/her own life.
